Indien HER2Neu positief:
1e lijn behandelschema: “TOGA”
Capecitabine, oxaliplatin met Trastuzumab.
Behandelschema:
Capecitabine + Oxaliplatin (3-wekelijks)
- capecitabine: 1000 mg/m2, 2 maal daags dag 1-14,
- oxaliplatin: 130 mg/m2, dag 1,
- Trastuzumab 1e dosering 8 mg/kg dag 1. Volgende doseringen 6 mg/kg (dag 1).
Na 6 cycli Trastuzumab monotherapie continueren.
Bij progressie Capox hervatten, geen evidence voor continueren trastuzumab na progressie.
indien HER2 negatief en CPS<5
Capecitabine + Oxaliplatin (3-wekelijks) (Cunningham 2008, Dijksterhuis 2019)
Indien in goede conditie (KI>90):
- Oxaliplatin,130 mg/m2,
- Capecitabine, 1000 mg/m2, 2 dd gedurende dag 1-14.
Behandeling gedurende 6-8 cycli.
Alternatief: Capecitabine monotherapie
Capecitabine 1250mg/m2 2 dd gedurende dag 1-14.
Bij passagestoornissen:
Overweeg gemcitabine / cisplatin (3-wekelijks) (van Zweeden 2018)
- gemcitabine 1250 mg/m2 (i.v. dag 1, 8),
- cisplatin 80 mg/m2 (i.v. day 1)
indien HER2 negatief en CPS>5
Capecitabine + Oxaliplatin + nivolumab (3-wekelijks) (Janjigian, Lancet 2021)
Indien in goede conditie (KI>90):
- Oxaliplatin,130 mg/m2 dag 1 IV
- Nivolumab 360 mg dag 1 IV
- Capecitabine, 1000 mg/m2, 2 dd gedurende dag 1-14 per os

2e lijns behandelschema:
Ramuciramb paclitaxel (4 wekelijks) “RAINBOW" (Wilke 2014)
- ramucirumab 8 mg/kg IV dag 1 en 15 plus,
- paclitaxel 80 mg/m(2) IV dag 1, 8, en 15.
Continueren tot progressie of onacceptabele toxiciteit.

Trifluridine/Tipiracil
Trifluridine/Tipiracil 35 mg /m2 dag 1-5 en 8-12 van 28 daagse cyclus.
Nadien
MSI bepalen voor evt studiebehandeling.

Indien in goede conditie (KI>90): CAPOX.
Behandelschema:
- Oxaliplatin, 130 mg/m2,
- Capecitabine, 1000 mg/m2 2 dd gedurende dag 1-14,
- Oxaliplatin + Capecitabine (CAPOX) Maagcarcinoom.
Capecitabine monotherapie
Alternatief is Capecitabine monotherapie.
Behandelschema:
- Capecitabine, 1250mg/m2, 2 dd gedurende dag 1-14,
- Capecitabine dagelijks Maagcarcinoom.
Passagestoornissen
Bij passagestoornissen kan combinatie gemcitabine / cisplatin behandeling worden overwogen als 1e lijns behandeling gemetastaseerd (gemcitabine 1250 mg/m2 (i.v. dag 1, 8) en cisplatin 80 mg/m2 (i.v. day 1) elke 3 weken)(5).
- Gemcitabine + Cisplatin 3-wekelijks Maagcarcinoom.
2e lijns behandelschema:“RAINBOW“ (6)
- Paclitaxel, 80 mg/m2, dag 1, 8, 15, 22,
- Ramucirumab, 8 mg/m2, dag 1 en 15.
In 4 weeks cyclus (28 dagen)
Paclitaxel + Ramucirumab Maagcarcinoom3e lijn
- TAS-102 – Trifluridine/Tipiracil (8)
Trifluridine/Tipiracil 35 mg /m2 dag 1-5 en 8-12 van 28 daagse cyclus.
Nadien MSI bepalen voor evt studiebehandeling.
